New Day Care Customers
Please complete and return prior to *pets*'s first visit (Meet & Greet-4 hour session in day care to meet attendants, other visitors, and get familiar with environment)

In addition to the above forms, Day Care visitors should also have the following forms:
1. Proof of Current Vaccinations: rabies, distemper/parvo, bordetella, influenza & leptospirosis
2. Proof of spay/neuter
3. Current flea control
Once the above information is received, verified, and reviewed (please provide 24 hours from the receipt of all forms), we will reach out to you to schedule the Meet & Greet.
During the meet and greet, your fur baby will have a chance to get to know our team and doggy pack. He/she will be introduced to our day care setting, as well as our daily routine (great for our anxious pack members), and the other pack members. Our Meet & Greet is a 4 hour session in our day care. Meet & Greet Prices are $20/$25
